What are the causes of erectile dysfunction?

Some common causes of erectile dysfunction are:

  • Vascular diseases
  • Hormonal causes such as due to impairments in the thyroid
  • Peyronie’s disease and other related anatomical issues
  • Drug and alcohol abuse
  • Psychological problems such as stress, anxiety and depression

How is erectile dysfunction diagnosed?

A physical examination, and recounting your medical history is usually sufficient for a diagnosis. During a consultation, your urologist will also go through the IIEF (International Index of Erectile Function) which is a routine questionnaire to help diagnose ED. In case your urologist suspects an underlying condition causing your dysfunction, they might suggest blood and urine tests which also check for your hormones, and in certain cases ultrasounds to check your blood flow or refer you for a psychological screening.

What are the treatments available for erectile dysfunction?

In case your urologist finds an underlying condition causing your erectile dysfunction, treatment will be given to address that issue or you will be referred to a specialist.

If this is not the case, listed below are the main treatments that are given in stages for erectile dysfunction. This means that your urologist will always recommend that you start with oral medications, and go through the stages and only get surgery when you have had no success otherwise.

  • Stage 1 - Oral medications: These include the popular sildenafil (commonly known as viagra), tadalafil, vardenafil. These medicines work by maintaining the effects of nitric oxide that is released when you are sexually stimulated. Nitric oxide produces a chain reaction which relaxes the smooth muscles in your penis, causing the arteries in your penis to dilate and blood to flow resulting in an erection. It is important to note that you have to already be sexually stimulated for the medications to work. Lastly, these medications are not recommended if you are taking nitrates.
  • Stage 2 - Intraurethral Alprostadil: In this form of treatment, you insert a suppository (which is a solid rounded dosage) into your urethra. The effects start within 10 minutes of applications and last until an hour.
  • Stage 3 - Intracavernous Alprostadil: This includes injecting the base of your penis with alprostadil. The effects last until an hour.
  • Stage 4 - Vacuum erection tube: The tube is placed over your penis, and using the pump attached the air is removed from inside the tube. This leads to the creation of a vacuum, which causes blood to flow into your penis, resulting in an erection. Then you will secure the erection with a tension ring around the bottom of your penis and can then remove the tube.
  • Stage 5 - Surgery: Surgery is performed by a urologist under general anaesthesia, and there are two basic types of penis prosthesis:
    1. Semirigid (malleable): The prosthesis is flexible, and consists of two rods that are placed on the shaft of your penis.
    2. Hydraulic (natural): The prosthesis is closest to a natural penis, it can be concealed and allows for a man to have an erection when he chooses.

What should I expect from this procedure?

In case your erectile dysfunction is not caused by an underlying condition, your urologist will always recommend starting off with oral medications,and use surgery as a last resort. It is vital that you take medication in the dosage that is recommended, overdosing can result in unfortunate side effects that may be harmful. Lastly, as with any treatment it is necessary to make lifestyle changes such as maintaining a healthy weight, attending counselling sessions if required, exercising regularly and avoid smoking and drinking too much.
