HOW DOES FUE HAIR TRANSPLANT WORK?

FUE (Follicular Unit Excision) is an innovative, increasingly popular hair transplant procedure where a surgeon manually harvests individual hair follicles from your donor area and then grafts them into your recipient area. FUE hair transplant is performed under local anaesthesia. The donor area is usually the back and sides of your head or other parts of the body, like your beard or chest, if the donor hair on your scalp is not sufficient. FUE hair transplant is amongst the most popular techniques as it allows the scalp skin to fully recover leaving little to no scarring.

Hair follicles naturally grow in groupings of 1 to 4 hairs, known as follicular units or grafts. During an FUE hair transplant, the surgeon will harvest grafts from a donor area and insert them into the target sites, areas where hair is thinning or has been completely lost. The donor area contains hair that is resistant to the hormone that is thought to contribute to the miniaturisation and eventual loss of a hair, dihydrotestosterone (DHT). When this hair is transplanted from the donor area to the target site, it keeps that resistance to DHT and should remain permanently implanted. The number of grafts extracted in one session ranges from 500 up to 5000.

What does an FUE hair transplant surgery involve?

An FUE hair transplant surgery is carefully planned based on individual needs, preferences and clinics’ practices. However, there are a few main stages that apply to all hair transplant procedures. Below is a step-by-step guide for FUE hair transplant surgery.

1. FUE hair transplant consultation

To find out if you are eligible for an FUE hair transplant, you can send us a few pictures of your head (from all angles: top, front, left, right, back). One of our hair transplant specialists will give you an initial assessment of the number of grafts required. You will be also asked questions regarding your overall health, including:

  • If you have any medical conditions
  • If you take any medication on a regular basis
  • When your hair loss started
  • If hair loss runs in your family

This will allow the doctor to understand the underlying causes of your hair loss and what procedure would best work for you. This initial assessment will also indicate a cost estimate for your FUE hair transplant. Our hair loss treatment option guide gives you insightful information on hair loss and what your options are to stop it.

2. Planning your visit to the clinic

Once you decide to go ahead with your FUE hair transplant procedure, we will schedule the operation on the date you prefer. Surgery typically starts in the morning or early afternoon, so it is recommended to fly the day before to take proper time to rest and run medical tests beforehand.

3. On-site consultation with FUE hair transplant surgeon

You will discuss with your surgeon what coverage you aim to achieve with your FUE hair transplant, also depending on the quality and amount of your donor area. Once you’ve decided, the doctor will draw a line that will mark the beginning of your new hairline.

4. Medical exams

At the clinic, you will have physical and blood tests done to make sure you are in good health to start FUE hair transplant surgery. General blood tests will also check for major infectious diseases to make sure you are healthy and sound to proceed with the treatment and to rule out any medical reasons that could be of any harm.

5. Preparation for surgery

Once tests and drawing of new hairline are done, you will receive local anesthesia for the donor area to be desensitized. During an FUE hair transplant, the donor area is usually shaved down (although there are surgeons who utilize the no-shave technique). If you are afraid of needles, don’t hesitate to let us know! We work with selected doctors who offer in needle-free anaesthesia (which usually comes with an additional charge).

6. FUE hair transplant procedure

Below a step-by-step breakdown of how an FUE hair transplant is performed:

a. Harvesting/extracting the grafts

Once the anaesthesia has taken effect, the surgeon will start surgery. Hair grafts are extracted manually one at the time and placed in a container with a storage solution that will maintain the grafts' vitality.

b. Making the incisions

After the extraction, the surgeon (or a team of expert technicians will use microblades and fine needles (between 0.6mm and 1.0mm in diameter) to create the incision sites that will receive the hair grafts. The incisions determine the direction of hair growth and will be angled in a consistent pattern for a natural look and density.

c. Implanting the grafts

Once the incisions are made, the hair grafts are inserted into the target sites. The whole procedure can last 4 to 8 hours and is performed on the same day. During the procedure, many patients read, take a nap, or watch TV. You will also be given breaks during the procedure.

There are surgeons who offer Platelet-rich plasma (PRP) treatment as a complementary procedure to FUE hair transplant surgery to promote hair growth and healing after the surgical procedure. PRP treatment is typically performed during the implantation or after the grafts are implanted.

7. Post-op check up

After your FUE hair transplant is done, your head will be covered in bandages and you will be given painkillers. If you’re staying abroad, you will spend the night in a nearby accommodation. The day after surgery, you will meet with the surgeon for a post-operative check up and will receive precise instructions to follow during your post-operative time.

What shall I expect from an FUE hair transplant?

Here’s a breakdown of what to expect from an FUE hair transplant, from right after surgery until one year later. For a more detailed overview of what to expect after an FUE hair transplant surgery, check our Post-op guide on hair transplant surgery!

FUE hair transplant regrowth timeline

Percentage of new hair regrowing over a year after a hair transplant surgery.

First 2 weeks

Your scalp and forehead may be tender, red, or swollen for the first 2 weeks after surgery, and you may notice pimples in the recipient area. All the scabs will fall off during the second week after your FUE hair transplant, many of them with hair. Don’t panic!

Week 2 - 8

During this time, 90% of your transplanted hair will fall off. This is a normal process, occurring in 95% of FUE hair transplant cases. Some of the natural hair located next to the transplanted grafts may fall off as well. This phenomenon is known as “shock loss”. Shock loss is a natural, transient process. The donor area will recover gradually as your hair grows back.

Month 2 - 6

Between the second and the fourth month there will be a stationary phase. Around 5% of new fine hair may begin to grow during this time. Between the fourth and the fifth month after your FUE hair transplant, 20% to 25% of the transplanted hair will start to grow.

Month 6 - 12

Between the sixth and the ninth month, most transplanted hair will grow (around 75%). At the same time, hair gets denser and thicker.
After the ninth month an additional 20% - 25% of new hair will grow. In some cases, it may take up to 18 months for the hair in the crown area to fully regrow.

Firstly working with Catherine and Quno was perfect for me as I was apprehensive and with her patience and knowledge, I answered most concerns initially that meant the process was less stressful. The transfers worked well as there was only me and one other going to the hotel and same clinic. The arrivals area isn't as intimidating as I was envisaging but it is still busy. Hotel amazing. Dr Yavuz was a gent and friendly and reassuring, with great English. The rest of the technical support had some English but there was always someone to help - make sure you ask and tell them if you are not happy. The process is long around 6 hours, so get comfortable and have bathroom breaks as I personally found staying comfortable most difficult. The anaesthetics make a difference, as it is painful at times but will be worth it. I am just a week after HT, yes not sleeping and sore donor area but the information provided and facebook makes me feel more reassured !! thank you all especially Dr Yavuz and Catherine I don't believe you can do anything better, it was a perfect service.

I can’t praise both smile clinic, and qunomedical (in particular Kate) enough. I love the personal touches I got, and felt completely at ease discussing everything with Kate. She was absolutely fantastic and I don’t think I would have booked had it not been for Kate’s service and knowledge on the whole process from start to finish.

I can’t speak highly enough of the service I received from start to end of the process. The transplant is an idea I have always toyed with but not had the money to do it in England, and never felt quite comfortable enough to use a foreign service because of the borrow stories you hear. I made enquiries with a couple of clinics before going off the idea a bit because, despite most clinics having lots of 4 and 5 star reviews, there just seemed to be too many which were 1/2/3 stars making me not want to risk. However after coming across qunomedical by chance and registering some half hearted interest online, my mind was made for me after Kate got in contact with me. She was knowledgeable, friendly and engaging, and spoke with complete confidence regarding the whole process. It was obvious she knew her stuff and wasn’t just trying to get a sale, and from losing interest in the process just a few weeks before, my mind was now made up that actually I should go ahead with something I’d always wanted to do, with absolute peace of mind that I was in really safe hands. This opinion never changed after I had booked and actually my guy instinct was proved to be 100% correct. I was sent all relevant information, Kate answered my many questions me in a timely manner whenever I asked anything, and I was called in the lead up to check how I was feeling, if I had prepared everything and just to make sure everything in general was ok. When I arrived in Istanbul I was met straight away for my private transfer, ferries to and from the hospital and back to the airport with absolutely no hassle. The hospital was clean and very hygienic. It made me feel completely at ease that they insist on blood tests before proceeding to do the transplant. I also particularly liked the fact I was never asked for mo yeh up front, just proof of my flight details to show I had booked to fly. Case managers contacted me via WhatsApp so I also have contacts before and after my operation at the hospital, and again they are always on hand whenever I have had any questions. I can’t praise both smile clinic, and qunomedical (in particular Kate) enough. I don’t usually leave reviews but felt inclined to because

I relied on reviews to help me decide on a clinic myself, and I hope my review is enough to convince others to go ahead with surgery if it’s something they would like done. Excellent service start to finish, I cannot fault it. I love the personal touches I got, and felt completely at ease discussing everything with Kate. She was absolutely fantastic and I don’t think I would have booked had it not been for Kate’s service and knowledge on the whole process from start to finish. show more

Everything ran so smoothly, from the time i was picked up from the airport to the time i was dropped back to the airport.

Everything ran so smoothly, from the time i was picked up from the airport to the time i was dropped back to the airport. Everyone was so welcoming and professional, including the taxi driver, the hotel staff and the team at Smile. All facilities were very clean and comfortable, including taxi, hotel and clinic where procedure was carried out. I was very happy with the service I received from Qunomedical. Staff were very assertive, providing extensive information of the procedure, with a quick response time to any queries I had. I would however suggest that the staff at Qunomedical become even more familiar with pre procedure preparation. When I sent in my pictures for assessment to see if I was able to get a hair transplant, I was told it was possible, and they would be extracting about 4000 grafts. When I got there, and they did a closer assessment, they found I had folliculitis in parts of the doner area. I was unfortunately unaware of this as I had no pain or discomfort in the area. This condition was the main factor for substantially decreasing the amount of hairs extracted on the day by nearly half, and we had to negotiate a higher hairline as a result. It may be a good idea for Qunomedical staff in the future, to advice/suggest to people to go see a dermatologist to ensure their skin is healthy before making the trip to get their hair transplant.

As for the staff, they are very good, kind and helpful. They really want to help as much as they could.

I waited for the cab driver inside the airport at a car rental agency. Then the driver took me with a group of patients and drove us to the hotel. The next day I got picked up in the afternoon by the driver again then went to the hospital. Afterwards, they took a blood sample for PRP and did the blood test. Then I got a consultation with the doctor which was kind to me and honest. He drew the hairline after a discussion we had. Then they shaved my hair and after that the surgery. The surgery went good from beginning till the end. I didn't feel much pain. I was only exhausted at the end of the surgery because of its long-time and blood loss. As for the staff, they are very good, kind and helpful. They really want to help as much as they could. During the operation time, a lot of them were periodically passing by to check on me and ask if I need anything. The two days after the operation the check-ups went well and the wash was also ok. All the facilities provided were good and comfortable. At the end It was a great experience. They [Qunomedical] are always replying on time and the staff is professional and speaks good English.

Do your homework know what you want. I was Norwood 5A so 3850 grafts got me good density at front, medium on the top and less at the crown. Accept that you will have to make compromises.

I was collected at airport as indicated. Hotel was nice but not Irish 5 star. More equivalent to 3 star. As advised everything was communicated effectively by WhatsApp. Procedure they have a language specialist per each person to guide. They took my blood later used for PRP. The doctor took a look at my hairline admittedly it was a bit higher than ideal but was advised that due to muscle structure they couldn’t bring it down much lower. Anyone getting it done should be aware of this. Procedure I was happy to let the technicians do it. Pain was minimal initially. They look after you very well. I was struggling after the anaesthesia wore off. They got the language specialist to see if I was ok. Aftercare do what they tell you. I’m not in much pain. Would be happy to recommend to anyone considering it. Do your homework know what you want. I was Norwood 5A so 3850 grafts got me good density at front, medium on the top and less at the crown. Accept that you will have to make compromises. So far I am very happy. Bluntly it’s what is described. I didn’t have unrealistic expectations I found it [Qunomedical] very helpful.

Otherwise everything is ok.
